چکیده

INTRODUCTION Breakage of the screws which may cause non-union or collapse of vertebral structures has been a complication of the pedicle screw fixation at a rate of 3-7 % [1-2]. Recently, flexible rods have been used for dynamic stabilization to avoid complications such as adjacent segment degeneration due to excessive stiffness of the spinal fusion by allowing intersegmental motion at the surgical level [3]. Thus, it is necessary to investigate the influence of the flexible rods on the breakage risk in the pedicle screw fixation. However, there have been few studies of the breakage risk in the dynamic stabilization system using flexible rods. In this study, the breakage risk in the pedicle screw fixation was analyzed using finite element method for the rods with various flexibilities.

Study of bone-screw surface fixation in lumbar dynamic stabilization.

BACKGROUND We aimed to use the animal model of dynamic fixation to examine the interaction of the pedicle screw surface with surrounding bone, and determine whether pedicle screws achieve good mechanical stability in the vertebrae. METHODS Twenty-four goats aged 2-3 years had Cosmic ® pedicle screws implanted into both sides of the L2-L5 pedicles. Twelve goats in the bilateral dynamic fixatio...

Biomechanics of grade I degenerative lumbar spondylolisthesis. Part 2: treatment with threaded interbody cages/dowels and pedicle screws.

OBJECT The authors sought to determine the biomechanical effectiveness of threaded interbody cages or dowels compared with that achieved using pedicle screw instrumentation in resisting Grade I lumbar spine degenerative spondylolisthesis. METHODS Thirty-three levels obtained from seven cadaveric lumbar spines were instrumented with cages or dowels, pedicle screw/rod instrumentation, or both. ...

Finite element analysis of 3 posterior fixation techniques in the lumbar spine.

This study compared the biomechanics of 3 fixation techniques: bilateral pedicle screw (BPS) fixation, unilateral pedicle screw (UPS) fixation, and UPS supplemented with translaminar facet screw (UPS+TLFS) fixation. The study was conducted in an L3-L5 finite element model. Three different finite element models were created by adopting different fixation techniques after removal of the left L3-L...

Comparative study of unilateral and bilateral pedicle screw fixation in posterior lumbar interbody fusion.

A prospective, randomized clinical study was performed to determine whether unilateral pedicle screw fixation was comparable with bilateral fixation in 1- or 2-segment lumbar interbody fusion. One hundred eight patients with lumbar degenerative diseases were randomly assigned to the unilateral (n=56) or bilateral (n=52) pedicle screw fixation group. Interbody fusion was performed in 1 or 2 leve...
